The Live program is a free service provided to the Mackay and surrounding region.
Eligibility
To be supported by the Live Well program, you must:
- Be aged 16-25 years
- Be homeless or at risk of homelessness
- Be living in any type of accommodation including: temporary accommodation, private rental, Department of Housing, community housing
Case Management
The Live Well Program provides person-centred case management utilising Outcome Star tool. This tool identifies 10 domains vital to young people securing and maintaining housing, which include:
- Motivation and taking responsibility
- Self-care and living skills
- Managing money and administration
- Social networks and relationships
- Drug and alcohol misuse
- Physical Health
- Emotional and mental health
- Meaningful use of time
- Managing tenancy / accommodation
- Offending
